Yes, I do worry about not being able to access medical help when needed, it's a bit nerve-racking. When we were travelling not far from the Tajik border I said to my guide that if something happened to me they should drag me across the river and leave me by the road on the Tajik side as I would be more likely to get help that way. I was only half joking!
