Pollution control:
The painting tries to show in a scary prediction what would happen in future, when pollution will increase to a level that the human-specie would need such an apparatus just to breath clean air but, to no avail, red blood droplets already in the system
The painting, as many others, depicted on the iron-doors of a local workshop. So, on any Saturday it would be the best day to explore the Florentine quarter when those doors are closed.

I do not recall the precise location of this graffiti, the Florentine quarter general location at around 32.056, 34.77 on the map.
